Shaping the Future of Care Leaver Transitions: Your Voice Matters

To the care-experienced community, social workers, personal advisors, and local authority partners: We need your insight.

As we look at the current landscape of supported living for care leavers, we have to ask ourselves: are we truly building homes, or just providing placements? To create a service that actually empowers young people as they transition to adulthood, we need to move beyond “standard” and start focusing on “essential.”

We are looking for honest feedback from:

Care-experienced adults & care leavers: What do you wish your first supported home had? What would have made you feel truly safe and prepared?

Social Workers & PAs: What are the biggest gaps you see in current provision? What makes your job harder when trying to settle a young adult?

Local Authorities & ICBs: What does a gold standard partnership look like to you?

The Big Questions:

What are providers currently missing? (Is it life skills, emotional support, physical environment, or something else?)

What small things make a big difference?

How can we shape a service that prioritises what is actually important to the person living there?

We aren’t looking for business as usual. We want to build something that reflects your lived experience and professional expertise.
